All routine services, including passport corrections and additional passport pages, are available by APPOINTMENT ONLY.
You must complete form DS-4085 and pay the US $82 fee to have visa pages added to a valid U.S. passport. Please note that extra pages can only be added to passports in good condition. This service is usually completed in one day.

How do I correct the biographical information in my passport?
You will need to fill out form DS-5504 (if the change is within one year of passport issuance) or form DS-82 (if the change is more than one year after passport issuance) and bring evidence that describes the details of a name change or other change.
- If your name has changed you must submit a marriage certificate or court order showing the change of name.
- If there is a change or an error in your recently issued passport you must submit the appropriate evidence showing the correct information (i.e. birth certificate or marriage certificate).
- All documents submitted for a name change or a data correct must bear the official seal of the issuing authority. Uncertified copies or notarized document cannot be accepted.
- The DS-5504 does not require a fee. If your circumstances warrant use of form DS-82, you will need to pay all applicable fees. The embassy will return your re-issued U.S. passport as well as any documentary evidence you submitted.
Lost or Stolen Passport?

If you have an urgent need to travel and your passport was stolen, you may come to the U.S. Embassy on Monday through Friday from 8:00 a.m. to 11:30 a.m. to apply for an emergency passport.
